Question #9 explained:
Which one of the following is NOT an indication to give antibiotics to a patient diagnosed with asymptomatic bacteriuria?
A. Pregnancy
-
-
- Pregnant women may require treatment for asymptomatic bacteriuria, because they have a higher incidence of progressing to actual UTI or pyelonephritis, as compared to the general population. This may of course lead to poor outcomes for the baby.
-
B. Diabetic patient CORRECT
-
-
- Being diabetic is not an indication to treat asymptomatic bacteriuria.
-
C. Invasive urologic procedure planned (e.g., TURP)
-
-
- TURP = transurethral resection of the prostate. It is reasonable to attempt to sterilize the urinary tract prior to an invasive urological procedure that cuts through mucosa, so as to reduce the chances for post-procedure infection.
-
D. Recent kidney transplant
-
-
- If asymptomatic bacteriuria progresses to a kidney infection in a recent kidney transplant patient, it can compromise the anastomosis and render the transplanted organ dysfunctional. For this reason, it is acceptable to give these patients antibiotics. The topic is discussed in depth in the IDSA guideline.
