GB is a 42 year-old female with DM-II. She presents to her PCP and is subsequently diagnosed with a lower urinary tract infection plus a skin and soft tissue infection. There are no cultures available, but the provider wants one oral drug to cover for E. coli in the urine and MRSA on the skin. Which one of the following drugs would be a potential option for GB?

A. Amoxicillin-clavulanic acid

      • Will not provide MRSA coverage.

B. Fosfomycin

      • Can cover both organisms, but in the United States it is only available as an oral product that can treat UTI so the skin and soft tissue infection coverage would be lacking here. 

C. Ciprofloxacin

      • Can cover both organisms, but is rarely indicated for treatment of MRSA infections due to resistance. This would be a poor choice.

D. Sulfamethoxazole-trimethoprim CORRECT

      • Bactrim has activity against community-acquired MRSA and E. coli plus it can be used for UTI or skin and soft tissue infection.
